How to Signpost Patients

Patients are eligible if they have:

  • BMI over 30, or
  • BMI over 27 (for those from BME backgrounds or with a weight-related health condition)

A digital information leaflet is also available to share with patients, explaining the local service, its benefits and how to sign up.

 

How it works?

  1. Patient easily registers online for the private Spirit weight management service.
  2. Spirit team contacts the patient to complete a questionnaire and outline next steps.
  3. Patient summary document requested to ensure weight loss medication is safe and suitable for patient. Fee payable to practice.
  4. Consultation arranged with a healthcare professional for prescribing and guidance.
  5. Medication and essential supplies (e.g., wipes, sharps box) delivered directly to the patient’s home.
  6. Updates provided to the GP practice including medication initiation and completion
  7. Ongoing support with education and optional monthly check-ins with healthcare professional.
  8. Further payment to practice via the GP federation at 6 months.
